The advantage to the electronic alteration is that it will usually leave a record of who made changes. The law requires that changes made to a patient’s medical records require identification of exactly who made changes, when they were changes were made and why such changes were made. Absent such notation is per se a violation of the medical record keeping standard of care for health care providers.
